History of the Hillingdon Hospital

The hospital’s origin was a workhouse infirmary built in 1838. In 1907, a separate female infirmary was added, and this healthcare facility was started to be managed by the Middlesex County Council in 1929, after which the site began to develop in 1932. 

In October 1940, the hospital was seriously damaged due to bombings though there were no casualties; however, the hospital was moved to temporary accommodation. The new Hillingdon hospital was opened in 1967, which included 7 new wards, pathology laboratories, operation theatres, an accident and emergency department, an imaging department, an outpatient department, occupational therapy, physiotherapy, single rooms, and a canteen. The healthcare facility operates with more than 3,500 employees (Hillingdon Hospital, 2015).

The PESTEL model, an abbreviation for Political, Economic, Socio-cultural, Ecological, and Legislative, focuses on those aspects that surround organizations and need to be analyzed closely.

 

Political Factors

Political factors have an important role in determining those factors impacting Hillingdon Hospital. That the hospital is operating in a country where Brexit is an aspect that has shaken the country’s entire economy. The Functions of The Grhselon Ulrich

Ulrich (1997) proposes a human resources management model allowing the creation of added value where four functions must be fulfilled and includes: 

Strategic partner: Process-oriented and long-term, contributes to developing and applying the business strategy, builds and reviews its processes in line with the business strategy; the role of HRM is to align HR strategy and policies with the organization’s strategy.

Administrative expert: manages the employment contract, legislative, administrative, and payroll (operational / process); the role of HRM is to design and deliver effective HR processes; this is the traditional role of HRM. 

Agent change: person-oriented, but in the long term, it is the long-term oriented activities that bring support to workers; the role of HRM is to develop the capacity for change, among other things, by identifying and promoting behavior that will support the firm’s competitiveness. 

Employee champion: short-term person-oriented; he supports employees on a daily basis; the role of HRM is to manage the expectations and daily problems of employees.

For Dietrich and Taskin (2016), “these different roles are not mutually exclusive, they combine. This combination, and therefore this multiplicity of roles, requires new skills within the HR function. HRM is supposed to ensure that it achieves satisfactory results by giving equivalent attention to the four roles it must take on”.

  • The Hillingdon Hospital represents the main hospital unit for care in the Lausanne area: 64% of hospitalized residents in the district are, in fact, in its departments. The Hospices Group is also a major player in the private market, with a 19% market share in the canton of Vaud.

They are distinguished in particular by a significant volume of cases treated presenting a high complexity, excellence in research, and a broad influence which results in strong attractiveness. 
  • Training and research are the foundation of innovation at Hillingdon Hospital. These strengths are strengthened by the establishment’s relations with its key players, the Faculty of Biology and Medicine at TLC and ICR.
 

Weaknesses

  • The hospital needs more room for operations in terms of management of the institution, although they benefit from a specific law in this area. The hospital is responsible for its financial balance by still being a state service. However, it needs to have a wide choice of its means (finances and human resources) in the context of competition. Furthermore, they do not benefit from legal personality.
  • The current efficiency of the hospital needs to be improved as the environment becomes more competitive. Although the healthcare institute has started the ambulatory shift, it still needs to be stepped up. Alternatives to hospitalization (outpatient surgery) should be developed, considering new funding methods and the resulting comparisons between health establishments. 
  • The hospital does not present a sufficiently structured and convincing unifying project vis-à-vis the facility’s departments and establishments, and employees’ feeling of belonging to the institution is uneven. 
  • Based on a proposal from the General Management, the Council of State has already decided, in May 2002, to undertake a reorganization of the hospital aimed at reducing the number of hierarchical levels and specifying the roles and responsibilities of the various levels in particular between general management and departments and between departments and services. The first stage of this reorganization was implemented in September 2002.
 

Opportunities 

  • The patient profile is changing significantly: aging of the population, migratory movements, and more informed and demanding patients. The practice of medicine is also changing with the introduction of new forms of care. These structural changes force the institution to adapt its responses to its environment and offer the best possible services. In this sense, it is a complete vector of evolution for the health organization. 
  • Patients “outside the region” represent nearly 16% of the care activity of the hospital. Although the hospital has a good location, this situation still needs to be sufficiently exploited by the institution, which could take advantage of it to increase its attractiveness in specific fields or pathologies. The Group’s strongest appeal is in the cantons of Ealing, Harrow, Buckinghamshire and Hertfordshire (around 5% of hospitalizations); on the other hand, market shares are only 3% for the other regions.
 

Threats

  • According to UKACR demographic forecasts, population growth will be strongest in districts where the hospital does not currently enjoy a strong position (Nyon, Rolle). However, the London Borough of Hillingdon and the surrounding regions of Ealing, Harrow, Buckinghamshire and Hertfordshire will stagnate. The new mode of financing by services as well as the new cantonal hospital organization giving more weight to future sector hospitals will cause an increase in competition between actors within the framework of a limited population pool. In addition, the concentration of specialized care in four sectors, as proposed in the health policy report, will ultimately lead to an increase in the capacities of these centers and will consequently modify the flow of patients from these hospitals to the reference university hospital.

Hence, human resource management signifies a major strategic factor for the organization’s successful future.

Bargaining power of suppliers

The bargaining power of Hillingdon Hospital is moderate. Among significant factors, medical equipment and physicians are considered to be a supply for the healthcare institute. The hospital has set up its own information technology system. In addition, the hospital has a highly sophisticated Medicare system, and there is high competition among the suppliers, resulting in price reductions and a positive indicator for the hospital. A huge base of suppliers is associated with the supply of products to Hillingdon Hospital, which means it is easier for the facility to switch suppliers. Since the hospital purchases supplies in huge volumes, it tends to reduce the bargaining power of suppliers.

 

Bargaining power of consumers

Current rivalry

For Hillingdon, the competition is moderate. The reason is that the industry’s size is huge, and the hospital has been operative for many years. Though many other healthcare facilities operating in the region tend to compete with Hillingdon, these rivals are less powerful. Hillingdon also provides these services in Mount Vernon Hospital in cooperation with the East & North Hertfordshire NHS Trust. Moreover, government regulations and legislation limit the competition level in the health industry; hence the intensity of existing rivalry could be higher.

 

Threat of substitutes

The primary substitute for Hilingdon is Royal United Hospital, which provides the same service level. The regions being covered and the number of physicians visiting the hospital is close to Hillingdon. Many other smaller health systems are operating in the region that is directly competing with Hillingdon. The threat of substitutes is low, and the hospital has the potential to expand its operations in other regions. Substitutes are performing low, and their service could be of better quality in comparison to Hillingdon Hospital. Additionally, there is a factor of higher costs to switch from the hospital and go for a substitute. Since Hillingdon Hospital offers high-quality service, the threat of substitutes is low.

 

The threat of new entrants

This threat is significantly low as the barriers to entry are incredibly high. No firm can easily enter the industry because of the highly sophisticated healthcare industry in the country. The hospital has hired several healthcare assistants trained and educated in Mental Health Nursing. When a patient in mental health care is admitted to the hospital, they can receive in-house care. Differentiating yourself from the point of view of advanced medicine implies making choices and setting priorities, particularly regarding the lines of research pursued. It is also a question of innovating in the framework of patient care in order to stabilize, maintain, and even develop the population pool (Cady et al., 2011).

In that case, the ethical manager knows not to be anxiety-provoking or catastrophic and to give confidence to his employees and teams to overcome difficult situations and move closer to objectives.

The Kurt Lewin Change Mode has three stages that address maintaining and retaining change in the healthcare institution. The Lewin model’s stages are:

The USA legalized the use of hemp (cannabis that contains lesser than 0.3% of THC, the psychoactive component in marijuana). Since then, it has been increasingly used in a variety of medications. CBD, or cannabinoids, has gained rapid popularity, offering significant health-related benefits. With claims of CBD having revolutionary health benefits in treating chronic pain and several other symptoms and diseases. However, with several small types of research underway, there is very little clinical proof of the benefits of CBD, whichever form it may be used in. This leads the arguments to its possible side effects on those who are using it.  

It is important to mention at this point that since the benefits have yet to be proven, there is also no solid proof of its side effects, and the component is in the initial stages of being researched currently. CBD side effects have been medically divided into three types:

1- The general side effects that are noticeable at some point by all.

2- Side effects are caused when CBD is taken in high quantities.

3- Side effects appear by taking substandard CBD products.

 

General Side Effects Of CBD

The general side effects or risks associated with CBD include

1- Digestive system-related issues, including diarrhea and loss of appetite

2- Changes in mental alertness or feeling of drowsiness

3- Mood swings such as agitation and irritability

 

Side Effects of Taking Higher Quantities of CBD

Some patients may use higher doses of CBD for a more extended period of time. Besides having severe long-term health risks and side effects, the higher dosage of CBD is ineffective in treating the symptom or diseases. A study revealed that when CBD is most effective when taken in the right dose, higher or lower than the appropriate dosage, it tends to make its healing properties ineffective. Intake of higher CBD doses poses the following side effects

1- Liver failure

2- Severe reactions occur when interacting with other medications occur

3- Internal injuries when mixed with depressants or alcohol

 

Side Effects Associated With the Use Of Sub Standard CBD Products

Substandard CBD products are referred to as products that are unreliable due to 1) the use of heavy metals and pesticides as wrong farming procedures in the harvesting of cannabis, 2) products containing a higher level of THC than recommended or allowed, 3) constitution of synthetic cannabinoids, and 4) holds other contaminants, e.g., bacteria, mold, etc.

In such a case, the health benefits and side effects of using such a product are unimaginable and can pose serious threats to the user.

An example is of using a CBD product that contains higher levels of THC may cause

1- Anxiety and paranoia

2- Dryness of the mouth

3- Redness and dryness in eyes

4- Increase in appetite

5- Lethargy and sleepiness

6- Impaired memory

 

Final Words

CBD is a safe product when prepared following the guidelines of the FDA. The side effects mentioned here are possible outcomes of the ingredients present in most Cannabidiol products and their impact on the human body. When used safely, CBD is the best cannabis-based product to use, compared to other components of cannabinoids. Since it is still being researched, we can expect a lot of new findings related to its health benefits and side effects in the future.

Obesity, flabbiness and type 2 diabetes are associated with low-grade inflammation. Among the new mechanisms, the link with intestinal bacteria seems more and more convincing. This intestinal microbiota would play a key role in triggering inflammation and insulin resistance via different mechanisms, such as the translocation of bacteria or even bacterial compounds with the development of metabolic endotoxemia. Certain intestinal bacteria could also contribute in a deleterious or, on the contrary, beneficial way to the improvement of carbohydrate homeostasis. Among the potential candidates, the role of Akkermansia muciniphila is currently being investigated.


Introduction


Obesity and overweight are classically associated with low-grade inflammation. This inflammation contributes to the development of insulin resistance, type 2 diabetes, and other cardio-metabolic complications. Over the past decade, many studies have associated the intestinal microbiota (formerly called: intestinal flora) with the development of these metabolic disorders (1,2). Over the past twenty years, our laboratory has contributed to a better understanding and elucidation of how the intestinal microbiota manages to dialogue with our body and contributes to the development of obesity and its associated metabolic disorders (insulin resistance, type 2, metabolic inflammation, non-alcoholic fatty liver disease (NASH)) (3-5)

 

Metabolic Endotoxemia


Among the candidates involved in this inflammation, we have proposed that constituents of the wall of intestinal bacteria (gram-negative), such as lipopolysaccharides (LPS) (also called endotoxins), would play an essential role in the triggering of some of these disorders (6 ). LPS are powerful pro-inflammatory molecules, continuously produced by the intestinal microbiota and whose absorption is directly linked to the ingestion of dietary lipids (3,7). Indeed, intestinal bacteria would contribute to the inflammation associated with metabolic disorders by mechanisms involving, in particular, an increase in plasma LPS levels, which we have defined as “metabolic endotoxemia” (3).

The existence of metabolic endotoxemia and its role in triggering inflammation and insulin resistance associated with obesity and type-2 diabetes was first demonstrated experimentally in animals. Still, it was later largely confirmed in humans (3.7-9). From an experimental point of view, we demonstrated that a chronic infusion of LPS mimicking metabolic endotoxemia induces inflammation and insulin resistance associated with hepatic steatosis.

In addition, the invalidation of the LPS receptor (CD14/Toll-like receptor 4 (TLR-4)) protects against the development of various metabolic disorders induced by LPS or a high-fat diet.

 

Dialogues Between Bacteria and Host Cells


More recently, we demonstrated that the established dialogue between gut bacteria and host gut cells plays a prime role in the development of obesity and diabetes. By specifically inactivating (in gut epithelial cells) an innate immune system protein called MyD881 can reduce inflammation, insulin resistance, and type-2 diabetes associated with a high-fat diet ( 10). This protection is directly dependent on the composition and activity of intestinal bacteria, thus suggesting that intestinal cells play a key role in the systemic response to components of the intestinal microbiota.

In this context, the barrier function of the intestine is essential in order to limit as much as possible the passage of undesirable compounds from the intestinal lumen to the blood circulation and the tissues of the host. The effectiveness of this intestinal barrier is ensured by different cell types and different mechanisms (tight junction proteins, mucus layer, antimicrobial proteins and immunoglobulins, etc.) (10).

Among the mechanisms potentially involved in the bacteria-host dialogue, we found that the gut microbiota interacts closely with the endocannabinoid (eCB) system and its bioactive lipids (11,12). Indeed, the eCB system is involved in the control of the barrier function of the intestine, and certain bacteria (or intestinal microbiota) would be associated either with protection or, on the contrary, with the triggering of disorders of the intestinal barrier for review (6).

More recently, we discovered that the eCB system in adipose tissue, specifically the enzyme for the synthesis of N-acyl ethanolamines (NAPE-PLD), plays a key role in the regulation of energy metabolism (11). This substance (enzyme) is involved in the synthesis of bioactive molecules, some of which are already known for their effects on inflammation and appetite regulation.

Using genetic tools, we discovered that eliminating the enzyme specifically in adipocytes leads to obesity and insulin resistance. This is associated with an almost complete disappearance of beige cells, thus indicating an inability to oxidize fat. The absence of NAPE-PLD in this organ also prevents the development of beige cells during exposure to cold, preventing the mice from expending energy to produce heat.

Our work shows that animals without NAPE-PLD in adipose tissue develop inflammation associated with metabolic endotoxemia. In agreement with this observation, the composition of the bacteria in the intestine of these animals is also different.

This surprising result, therefore, suggests that adipose tissue interacts with the intestine and bacteria. Our work suggests that certain bioactive lipids may modify metabolism through a microbiota-host metabolic dialogue. But this dialogue does not only take place in the direction of adipose tissue to the intestine. Indeed, transferring gut bacteria from these mice into axenic animals causes a decrease in browning/beiging and fat oxidation, thus suggesting that gut bacteria would be able to control the metabolism of adipose tissue.

A place of choice for certain candidates?

Akkermansia Muciniphila


Various studies have shown an association between intestinal microbiota composition, body weight, hyperglycemia, and type-2 diabetes. Many metagenomic analyzes show that certain bacteria or bacterial families. They increase or decrease in the feces of type-2 diabetic patients compared to non-diabetic individuals. Still, there is no reliable “listing” to suggest a causal link between these observations and the onset of diabetes.

On the other hand, there is another bacterium called Akkermansia muciniphila which is interesting in the context of metabolic disorders. Indeed, this bacterium has been associated with energy and carbohydrate metabolism on several occasions. For example, we found this bacterium was less present in obese and type 2 diabetic mice, regardless of the model, i.e., genetic or nutritional (13,14). Next, we demonstrated that the administration of Akkermansia muciniphila to obese and diabetic animals reduced body weight gain, improved blood sugar and insulin resistance, strengthened the intestinal barrier, and decreased metabolic inflammation (14); other teams have recently confirmed these observations (15,16).

In humans, various studies have shown that the presence of Akkermansia muciniphila was inversely correlated with body weight or blood sugar (17-19). Note, however, that the abundance of this bacterium is increased by taking metformin, which makes it a confounding factor during its investigation in the intestine of type-2 diabetic patients (20,21).

Akkermansia Muciniphila

Our recent work has shown that the abundance of Akkermansia muciniphila could predict a patient’s response and metabolic improvements following a low-calorie diet. Specifically, subjects with more Akkermansia muciniphila before the nutritional intervention will show the greatest improvement in insulin sensitivity, greater decrease in total and LDL cholesterol, and waist circumference. Akkermansia muciniphila was also inversely correlated with fasting blood glucose but also with other parameters, such as the hip-to-waist ratio and the size of adipocytes in subcutaneous adipose tissue (17).

To date, no intervention study has been able to demonstrate whether this bacterium has any potential health benefits. This hypothesis is currently being investigated at the Cliniques Universitaires Saint-Luc in collaboration with Professors Jean-Paul Thissen, Michel Hermans, Dominique Maiter and Doctor Audrey Loumaye.

In conclusion, a lot of work is underway, and over a relatively short period of a decade, a significant number of advances have been made. Obviously, the influence of the intestinal microbiota on carbohydrate and energy homeostasis is complex and multifactorial. However, some leads could be suggested for the specific management of metabolic syndrome. Current research encourages the development of new therapeutic targets that will be personalized. Both targets, such as immunity or even different bacterial metabolites, are of particular interest.
